Hypnobirthing Group Series

Cancellations prior to 1 week before our class start time will incur a $100 fee. There are NO REFUNDS if you cancel within 1 week of our class start date or after our class has begun.
If you birth early or have an unusual emergency, please contact me to discuss your situation on a case by case basis. Mind Body Baby reserves the right to cancel any class or class series at any point prior to the class start date and will refund all monies paid to all registrants. Your signature below is confirmation that you understand and agree to the refund policy stated above.

I hereby state that I am enrolling in the Hypnobirthing class of my own free will and with the understanding that this is a program designed to teach me to use my own natural abilities to bring my mind and my body into a state of relaxation. I further understand that the content of these classes is in no way intended to be represented as medical advice or as a prescription for medical procedure. I am aware that I should seek the advice of a health-care provider to answer any health-related or pregnancy-related issues surrounding my pregnancy, my labor, or my birth.

I therefore agree that I will in no way hold Mind Body Baby LLC or its representatives responsible for any special circumstances that could arise as a result of my pregnancy, my labor, or the birth of my child; and I agree that neither I nor any member of my family will make any claim or initiate any suit against any of the above-named parties now or at any time in the future.

MISSED/MAKEUP CLASS POLICY
It is vitally important to your Hypnobirthing education that you and your birthing companion attend each scheduled class, so please make absolutely sure you can attend each of the sessions before signing up. Hypnobirthing is a tailored series to fit each of the preplanned topics into five classes, and each class builds upon the previous one. Being able to 'make up' a class will not be possible due to the personal nature and limits of the group as well as the five part structure. Should an emergency arise please reach out to see if there are any possible alternate arrangements that can be made to get you up to speed for the information that was missed. No refunds will be given for missed classes.

In the event the instructor needs to miss a class due to an unforeseen circumstance (such as a family emergency or to attend a birth), we will alert all students as soon as possible, and arrange a make up session at no extra charge. Of course, we will do everything in our power to avoid such situations and to always keep open communication with students.

PHOTO RELEASE
I hereby grant permission to Mind Body Baby LLC to use photographs and/or video of me taken during our Hypnobirthing series in publications, news releases, online, and in other communications related to the mission of Mind Body Baby LLC.

COVID-19 WAIVER FOR IN PERSON CLASSES
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.

I further acknowledge that Mind Body Baby Orange County has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that Mind Body Baby Orange County can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, other students and their families.

I voluntarily seek services provided by Mind Body Baby Orange County and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending sessions.

I attest that:
I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

I have not traveled internationally within the last 14 days.

I have not traveled to a highly impacted area within the United States of America in the last 14 days.

I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.

I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold Mind Body Baby Orange County harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Mind Body Baby Orange County. I understand that this release discharges Mind Body Baby Orange County from any liability or claim that I, my heirs, or any personal representatives may have against the business with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Mind Body Baby Orange County. This liability waiver and release extends to Mind Body Baby Orange County together with all partners and employees.
